Family Model Provider

Caregiver Inc. is seeking compassionate and dependable Family Model Providers to support individuals with intellectual and developmental disabilities (IDD) in a welcoming, home-based environment. Responsibilities:

• Provide individualized support with daily living activities, including personal care, grooming, dressing, and mobility as needed.
• Promote independence, personal choice, dignity, and community involvement.
• Assist with meal planning, preparation, and maintaining a safe, clean-living environment.
• Support medication routines in accordance with established procedures and individual care plans.
• Monitor and communicate changes in health, behavior, or support needs.
• Follow individualized service plans and applicable policies, procedures, and confidentiality requirements.
• Encourage participation in meaningful activities, social opportunities, and community outings.
• Maintain accurate documentation and communicate effectively with the CHS Group team.

• Monthly compensation ranging from $1,600 to $6,000, depending on the applicable arrangement and support requirements.

Work Setting: In-person, home-based support

Qualifications:

• High school diploma or GED required.
• Valid driver's license required.
• Ability to successfully pass a required background check.
• Strong communication, organization, and problem-solving skills.
• Compassion, patience, reliability, and a genuine desire to support individuals with disabilities.
• Previous experience in caregiving, direct support, home health, or IDD services is preferred but not required.
• CPR, First Aid, and AED certification required; training is provided by CHS Group TN, LLC.
